QUALITY IMPROVEMENT

A team of representatives from the NYS Dept. of Health, American Cancer Society, NYC Dept. of Health and Mental Hygiene, CHCANYS and the Health Care Network of New York continue to work together to develop helpful tools and resources to assist health care practices in increasing colorectal cancer screening rates in support of the national 80% by 2018 initiative.  This webinar will provide information about patient navigation in two types of settings -- primary care and an endoscopy center -- and explain how patient navigation can be supported if resources are limited. Participants will also be introduced to NCCRT’s “Paying for Colorectal Cancer Screening Patient Navigation Toolkit,” designed to support health care professionals at every stage of a navigation program plan for sustainability.  To register, click here.
