Care Coordination/Patient Navigation Training Demo

 

CHCANYS is happy to announce an informational session on the Care Coordination/Patient Navigation training that it offers through a partnership with the 1199/SEIU Training Funds. In an effort to better prepare community health centers to offer patient-centered care, 1199TEF has developed a 48-hour training program. The program includes 24 two-hour modules that address a variety of crucial Care Coordination topics, including PCMH models, the basics of common chronic diseases, health coaching, and professional roles/conduct in care teams. During this informational session, we plan to discuss the program in more detail and present sample elements of the training content. At the end of the session and consultation back at your health center, we hope you will decide to schedule a training for your staff. 

To demonstrate the scope of the training program, CHCANYS will host an informational session at our offices on 8th Avenue on Friday, April 5th from 7:30am to 9am. The session will begin and end on time and will include training content from the first two modules. This training could be invaluable as patient-centered models become critical for community health centers, and CHCANYS is dedicated to ensuring that the FQHC workforce receives all training possible to enhance care coordination.
