Nonmedical determinants disproportionately affect population health. The evidence is clear that where people live, eat, work, and play directly affects their health outcomes. In response, the health care and social service sectors have coordinated a wide range of activities to bridge health care and population health activities. Started in 2013, the annual Population Health Summits have focused on financing population health, bridging efforts between health care and population health, and addressing the social determinants of health in population health. This year, the fifth annual Population Health Summit will look back on the previous summits to assess our advances and stumbling blocks, as well as the field's readiness to address what lies ahead. Given the threats to the Affordable Care Act and the crisis of the opioid epidemic, the time to reflect thoughtfully and forge ahead with population health efforts is now. The summit will feature a diverse set of perspectives from national, State, and city leaders who will share their strategies for tracking progress, lessons learned, and opportunities to expand and advance population health efforts in New York State. Panel sessions will focus on the continuum of strategies to address broader determinants of health, as well as the multiple approaches underway to tackle the opioid epidemic. The summit will also provide an opportunity for attendees to share their population health work at a poster session. Health care and social service providers, insurers, policymakers, public health and community leaders, business leaders, and researchers should attend this conference.
