Self Management Training Track
In response to CHCANYS’ Clinical Committee Survey and HRSA Quality Inventory results, we are excited to announce a special Self-Management training day hosted in partnership with the NY State Department of Health’s Diabetes Prevention and Control Program as well as the Healthy Heart Program. The training will be held on January 23, 2008 from 10:00 am to 4:00 pm at the Hilton Garden Inn at Albany Medical Center, 62 New Scotland Avenue, Albanys,New York 12208. Guidance for selecting staff to attend: Consider sending a staff member who can bring back information to support training of additional staff back at the organization on the topic of self-management. A person tasked to support workforce development and can link the training to existing improvement initiatives within the organization. The following topics on self-management will be covered:
Self-management Overview The training day is FREE and participants will receive CME/CEU for their participation. We are able to accommodate approximately 75 participants from health centers across the state. Please complete the online registration and depending on the level of need across the state, we might need to limit the number of slots per site to TWO. CHCANYS will continuously evaluate the registration list to ensure equitable distribution of attendee slots.
